Shoulder and axillary wounds represent one of the most challenging hemorrhage control scenarios in TCCC: the injury is in the junctional zone where a standard extremity tourniquet cannot be applied, the wound cavity is complex and irregular, and improper packing technique — too superficial, too little pressure, or gauze packed in the wrong direction — fails to achieve hemostasis with potentially fatal consequences. CoTCCC guidelines address junctional hemorrhage through wound packing with hemostatic agents and pressure, but teaching effective wound packing technique requires a trainer that accurately represents the anatomy and provides clear feedback on packing efficacy. The trainer features two gunshot wound sites (entry and exit) and one large internal cavity that simulate the wound geometry of a high-velocity penetrating shoulder injury. Internally attached bleeding tubes simulate major hemorrhage, and the bleeding stops visually when packing achieves sufficient depth and pressure to occlude the simulated vessel — providing the immediate feedback that confirms technique. Students learn to pack hemostatic gauze into the wound cavity to the base, maintain pressure, and recognize when hemostasis has been achieved, all on a trainer that responds to correct technique rather than accepting any arbitrary packing depth as sufficient.
