Product Overview

ChitoSAM 100 stands apart from other chitosan hemostatic products in a critical way: where most chitosan-based hemostatics (Celox Rapid, ChitoGAUZE XR PRO, Axiostat) use a gauze substrate coated or impregnated with chitosan, ChitoSAM 100 is the gauze itself — spun directly from chitosan derived from snow crab shells. The result is a 100% pure chitosan non-woven fabric where every fiber, every surface, and every layer is chitosan. There is no substrate to separate from the active agent, no coating to wash off, and no non-hemostatic matrix diluting the chitosan contact area. This maximizes the chitosan-to-wound-surface contact ratio and potentially enhances hemostatic efficacy in the critical seconds after application.

Like all chitosan hemostatics, ChitoSAM 100 works independently of the body's clotting cascade. The positively charged chitosan fibers interact directly with the negatively charged red blood cell membranes, causing rapid aggregation and forming a physical hemostatic plug. This mechanism functions in hypothermic patients, anticoagulated patients (heparin, warfarin, DOACs), and those with trauma-induced coagulopathy — making ChitoSAM 100 a versatile option across diverse patient presentations. Bleeding control is typically achieved within minutes of application with appropriate pressure.

A distinctive product feature is SAM Medical's Tear Anywhere packaging — the waterproof sterile pouch can be torn open along any edge, improving single-hand access under stress and in low-light conditions where finding a specific tear notch can be difficult. The 3 in. × 6 ft. Z-fold format (CT100-C-EN) provides extended-length packing capability — longer than the standard 3 in. × 4 ft. (CT100-B-EN) — for larger wound cavities. SAM Medical is based in Portland, Oregon, the manufacturer of the SAM XT Extremity Tourniquet, bringing the same commitment to high-performance, clinician-designed medical devices to the hemostatic gauze market. The gauze is removed from wounds with water or saline solution, leaving no residue.

How to Apply

  1. Expose the wound and identify the bleeding source.
  2. Open the Tear Anywhere packaging — tear on any edge for rapid access.
  3. Remove the Z-fold ChitoSAM 100 gauze and apply one end directly to the bleeding source.
  4. Pack tightly into the wound, feeding in the gauze while maintaining pressure at the source.
  5. Apply firm, continuous direct pressure for 3–5 minutes or until bleeding stops. Do not lift to check.
  6. Secure with a pressure bandage and monitor during transport.
  7. Remove at definitive care using water or saline irrigation.

ChitoSAM 100 vs. Coated Chitosan Gauze

ChitoSAM 100's pure-spun chitosan construction differs from products like ChitoGAUZE XR PRO (chitosan-coated polyester/rayon gauze) and Celox RAPID (chitosan granules bonded to gauze). All three use chitosan's muco-adhesive mechanism and work independent of the clotting cascade. ChitoSAM 100's 100% pure chitosan structure maximizes active surface area; ChitoGAUZE XR PRO adds X-ray detectability and antimicrobial data; Celox RAPID's Chito-R bioadhesive reduces compression time to 60 seconds. CoTCCC-recommended QuikClot Combat Gauze (kaolin) remains the U.S. DoD primary hemostatic.

CLINICAL RATIONALE

Clinical Evidence & Training Rationale (MED-TAC)

  • Mechanism (the “why”): Chitosan carries a positive charge that binds negatively charged RBCs and tissue surfaces, forming an adhesive, stable clot at the source. This physicochemical mechanism works when the normal clotting cascade is impaired (hypothermia, anticoagulation, shock).
  • Material advantage: ChitoSAM™ 100 is non-woven, 100% chitosan spun fiber (not just a coated gauze), maximizing contact surface and conformability for wound packing and tamponade.
  • Operational design: Z-fold formats (3"×4′ and 3"×6′) support rapid, deliberate feed into wound tracks; “tear-anywhere” packaging reduces time-to-compression under stress.
  • Labeled indications & precautions: Temporary external dressing for control of bleeding; apply 3 minutes direct pressure; remove within 24 hours; shellfish-derived chitosan—consider allergy history; do not place over eyes.
  • Regulatory traceability: Example UDI-DI for 3"×6′ model CT100-C-EN: 10822045000008 (AccessGUDID).

Training notes (MED-TAC): Expose → identify source → aggressively pack with chitosan gauze → apply direct pressure for ≥3 minutes → secure pressure dressing. If rebleed: add additional gauze on top (do not remove initial pack); re-compress and reassess perfusion. Integrate within MARCH priorities and current hemorrhage-control algorithms (use a tourniquet first for extremity life-threatening hemorrhage when indicated).
