Intraosseous (IO) vascular access is a critical skill in both tactical and civilian emergency medicine — it provides rapid vascular access for fluid resuscitation and medication delivery when standard IV access fails or is not feasible in the timeframe required. The B.I.G. (Bone Injection Gun) is an automatic spring-loaded IO device that delivers the needle to the proximal tibia with a "position and press" mechanism. Like all procedural skills, effective B.I.G. use requires hands-on repetition to build the muscle memory needed for reliable execution under stress.

B.I.G. Device Training Protocol

  1. Palpate the proximal tibia on the training leg — locate the tibial tuberosity and move 1–2 cm medially and 1–2 cm distally to find the flat medial surface of the proximal tibia.
  2. Ensure the insertion site is 90° perpendicular to the bone surface.
  3. Place the B.I.G. device firmly against the skin at 90° with the arrow pointing down.
  4. Stabilize the device and use the palm to press down firmly, deploying the needle.
  5. Remove the B.I.G. housing by pulling upward in a slight side-to-side motion while maintaining needle stabilization.
  6. Confirm placement by noting correct depth, aspirating marrow, and testing fluid flow.
  7. Secure the needle and tape in place before fluid infusion.

Applications in Military and Tactical Medical Training

IO access is a recognized skill in TCCC for combat medics (18D, SARC, PJ, IDC) and tactical medics who must establish vascular access on casualties with collapsed or inaccessible peripheral veins.
