The Committee on Tactical Combat Casualty Care (CoTCCC) recommends specific devices for prehospital limb tourniquet use based on clinical evidence, field performance data, and standardized testing. For limb tourniquets, the three CoTCCC-recommended devices are: the Combat Application Tourniquet (CAT) Gen 7 manufactured by North American Rescue, the SAM XT Extremity Tourniquet manufactured by SAM Medical, and the SOF Tactical Tourniquet Wide (SOFTT-W) manufactured by Tactical Medical Solutions. Current TCCC doctrine and most tactical medicine programs recommend carrying a minimum of two tourniquets per operator in a tactical environment — one for immediate access (on the body, belt, or carrier) and one for buddy aid or self-aid to the opposite limb. Is the CAT Gen 7 different from the CAT Gen 6?+
Yes — the CAT Gen 7 (introduced 2020) includes several improvements over Gen 6: a reinforced windlass rod (stronger polycarbonate composite), an improved flip-clip windlass retention mechanism, updated strap material, and packaging changes. The Gen 7 is the current CoTCCC-recommended version and is the standard for new procurement across U.S. military and law enforcement. Gen 6 tourniquets remain effective if properly maintained, but Gen 7 is recommended for new purchases due to the mechanical improvements. MED-TAC sells current-generation Gen 7 only.
How do I apply a tourniquet one-handed to myself?+
For a leg: loop the tourniquet around the thigh, thread the tail through the buckle, pull tight with your teeth or free hand, then tighten the windlass until bleeding stops. For an arm: loop around the upper arm, route the tail through the D-ring, cinch down, then tighten the windlass. The key detail: apply 2–3 inches above the wound (proximal), not over a joint. Tighten until bleeding stops — pain on application is normal and indicates effective pressure. Mark application time with a marker. What is the TRUFORCE buckle on the SAM XT and why does it matter?+
The TRUFORCE buckle on the SAM XT provides an audible click and tactile feedback when the strap has been tensioned to the level needed for effective arterial occlusion. On standard windlass tourniquets, the operator must use tactile feedback from the limb or visual cues (cessation of bleeding) to confirm adequate tightening. Under stress, hypoxia, or in low-light environments, the TRUFORCE feedback gives a more definitive indication that the strap tension is adequate before windlass tightening begins — reducing the risk of undertightening, which is the most common cause of tourniquet failure. It's particularly valuable for self-application and for responders in high-stress situations.
Can tourniquets be used on children?+
Standard windlass tourniquets (CAT Gen 7, SOFTT-W) can be applied to adolescents and larger children whose limb circumference allows adequate strap tension and windlass rotation. For smaller children, the SWAT-T's elastic wrap design or pediatric-specific tourniquets are more appropriate — windlass tourniquets on small limbs may not achieve vascular occlusion.
