Manual auscultatory blood pressure measurement using a calibrated aneroid sphygmomanometer and stethoscope remains the reference standard in situations where noise is manageable and provider training is assured. In prehospital trauma assessment, trending blood pressure over time from scene to transport is a critical hemodynamic monitoring tool. A falling systolic BP signals progressive hemorrhagic shock; a target systolic of 80–90 mmHg in penetrating torso trauma (permissive hypotension) is only achievable if BP can be accurately measured and tracked — which requires a reliable manual or digital measurement tool.
