The NIO Pediatric bridges this anatomical gap with a semi-automatic spring-loaded design and an innovative adjustable penetration depth system that accommodates the full 3–12 year age range with a single device.
The adjustable penetration depth is the NIO Pediatric's most clinically significant feature. A removable red spacer adjusts the device's stop depth from 14 mm (appropriate for the smaller tibias of younger pediatric patients, ages 3–9) to 18 mm (appropriate for the approaching-adult tibial anatomy of older pediatric patients, ages 9–12). The provider checks the child's age before activation and either leaves the spacer in place or removes it — a one-step depth calibration that eliminates the need for separate pediatric sizing devices.
The needle stabilizer hub serves four functions: it adjusts penetration depth, positions the insertion arrows for tibial tuberosity landmark guidance, mechanically limits depth to prevent over-penetration, and stabilizes the needle against the limb post-insertion to prevent rotational dislodgment. NIO Pediatric Insertion Sequence
- Check patient age: 3–9 years (leave red spacer) / 9–12 years (remove red spacer)
- Identify proximal tibia: palpate tibial tuberosity; needle insertion site is 1–2 cm below on medial flat surface
- Unlock NIO: rotate cap 90° so cap aligns with trigger wings
- Position NIO at 90° to insertion site; stabilize with non-dominant hand applying downward pressure
- Align NIO arrows toward tibial tuberosity
- Place dominant palm over cap; apply downward pressure AND squeeze trigger wings simultaneously
- Remove stylet/cap; connect 90° IV extension set from Go IO kit
- Confirm placement: aspirate marrow/blood; saline flush with no subcutaneous infiltration
- Apply fixation dressing
