It differs from needle decompression in that finger confirmation of pleural entry is possible, making it more reliably effective in patients with difficult anatomy or chest wall variability.

Why is the HyFin Vent Compact Chest Seal included in the thoracostomy kit?

After simple thoracostomy, the open pleural entry site requires management in spontaneously breathing patients. The HyFin® Vent Compact Chest Seal (vented) is applied to the site to provide occlusion while allowing continued air venting — preventing the site from becoming an open chest wound that could allow atmospheric air ingress during inspiration. The vented design is critical: a non-vented (occlusive) dressing placed over an open thoracostomy site could cause re-accumulation of tension. The Twin Pack allows coverage of bilateral sites or provides a backup seal.

What is the purpose of the Chloraprep swabsticks in a tactical kit?

Chloraprep® (chlorhexidine gluconate/isopropyl alcohol) swabsticks provide rapid antiseptic skin preparation to reduce infection risk at the thoracostomy incision site. The kit includes a 3-pack, which provides sufficient prep for bilateral procedures — an important design consideration given that TCCC protocols may require bilateral decompression in casualties with torso trauma who are in cardiac arrest. In austere environments, infection risk from non-sterile field procedures is a significant cause of morbidity, making antiseptic prep a best practice even under time pressure.
