What It Is
The nasopharyngeal airway is the preferred first-line airway adjunct for the semi-conscious casualty, because — unlike an oropharyngeal airway — it does not require an absent gag reflex. The Sunmed adjustable-flange NPA is made from soft Neoprene that conforms to nasal anatomy, with an atraumatic tip to reduce mucosal trauma. The adjustable flange lets the depth be set to the patient, then locked, helping prevent migration during movement and transport.
Contraindications
- Suspected basilar skull fracture — Battle's sign (retroauricular bruising), raccoon eyes (periorbital bruising), or clear fluid (CSF) from ears/nose. Risk of intracranial placement.
- Significant midface trauma disrupting the nasopharynx or skull base.
- Known facial/nasal fracture in the insertion path.
- Suspected epiglottitis
- Large nasal polyps or obstructing nasal pathology
- Recent nasal or sinus surgery
- Coagulopathy or anticoagulant therapy (increased epistaxis risk)
Per 2024 TCCC guidance, an unconscious casualty without traumatic airway obstruction is placed in the recovery position to maintain patency; the NPA remains a first-line prehospital airway adjunct prior to surgical-airway consideration.
