Adjustable Flange Nasopharyngeal Airway

A latex-free NPA with an adjustable flange for precise depth positioning — for the obtunded or semi-conscious casualty.

What is the Adjustable Flange Nasopharyngeal Airway? A single-use NPA by Sunmed made from Neoprene—latex-free, with atraumatic tips for smooth nasal passage. The adjustable flange allows precise depth positioning for comfort and fit. Available in 22, 24, 26, and 28 French. A CoTCCC-recommended class of airway adjunct for trauma casualties with airway obstruction or altered consciousness.

Provider scope

NPA sizing, contraindication screening, and placement are trained clinical skills. This page describes the device, its sizing, and its contraindications — it is not a substitute for accredited airway training, and is not a procedural how-to.

What It Is

The nasopharyngeal airway is the preferred first-line airway adjunct for the semi-conscious casualty, because — unlike an oropharyngeal airway — it does not require an absent gag reflex. The Sunmed adjustable-flange NPA is made from soft Neoprene that conforms to nasal anatomy, with an atraumatic tip to reduce mucosal trauma. The adjustable flange lets the depth be set to the patient, then locked, helping prevent migration during movement and transport.

Contraindications

Absolute — do not place an NPA:
  • Suspected basilar skull fracture — Battle's sign (retroauricular bruising), raccoon eyes (periorbital bruising), or clear fluid (CSF) from ears/nose. Risk of intracranial placement.
  • Significant midface trauma disrupting the nasopharynx or skull base.
  • Known facial/nasal fracture in the insertion path.
Relative — use with caution:
  • Suspected epiglottitis
  • Large nasal polyps or obstructing nasal pathology
  • Recent nasal or sinus surgery
  • Coagulopathy or anticoagulant therapy (increased epistaxis risk)
TCCC field note

Per 2024 TCCC guidance, an unconscious casualty without traumatic airway obstruction is placed in the recovery position to maintain patency; the NPA remains a first-line prehospital airway adjunct prior to surgical-airway consideration. See the TCCC Guidelines 2026 overview and the full airway management collection.
