The BAC-Pack™ (Bougie-Aided Cricothyroidotomy Pack) from North American Rescue is designed for the most critical airway scenario in tactical and pre-hospital care: a "can't intubate, can't oxygenate" situation requiring a definitive surgical airway. When conventional airway management — NPA, OPA, supraglottic airway, BVM ventilation — has failed due to massive facial trauma, airway obstruction, or anatomical factors, cricothyroidotomy provides a direct surgical airway through the cricothyroid membrane.
The BAC-Pack's defining innovation is the pre-inserted bougie. Traditional surgical cricothyroidotomy requires the provider to perform the incision, then separately introduce a bougie through the membrane opening, then railroad the tracheal tube over the bougie. The BAC-Pack eliminates the separate bougie introduction step by pre-loading the bougie into the cuffed tracheostomy tube — the provider incises the membrane, inserts the bougie tip directly through the incision, then advances the tube as a single unit. This reduces the number of hand movements and steps in a procedure that must be performed rapidly under extreme stress.
The tube meets all TCCC specifications for the bougie-aided open surgical technique: flanged and cuffed airway cannula with less than 10 mm outer diameter, 6–7 mm inner diameter, and 5–8 cm of intratracheal length. The bougie is fully adjustable based on patient anatomy, and excess bougie stores in the purpose-built bougie cap/stopper. The #10 Protected Scalpel adds a safety feature — the blade is covered until the moment of use, reducing inadvertent laceration during high-stress deployment.
Why Bougie-Aided?
The bougie-aided approach improves first-pass success rates in surgical cricothyroidotomy by providing a directional guide for tube placement. The bougie is first advanced through the incision into the trachea; the tube is then railroaded over it into position.
