What is the i-gel Resus Pack? The i-gel® Resus Pack is a second-generation supraglottic airway (SGA) device from Intersurgical, designed for EMS and emergency resuscitation applications. Made from medical-grade thermoplastic elastomer (TPE) — not inflated — the i-gel creates a non-inflatable anatomical seal of pharyngeal and laryngeal structures without cuff inflation. Available in Size 3 (Small Adult, 30–60 kg), Size 4 (Medium Adult, 50–90 kg), and Size 5 (Large Adult, 90+ kg). Each pack includes the i-gel, lubricant sachet, airway support strap, and 12Fr suction tube. Advanced provider — TACEVAC/Prolonged Field Care level skill.

Product Overview

The i-gel® is one of the most clinically studied supraglottic airway devices in emergency medicine. Launched in 2007, it features a non-inflatable cuff made from soft thermoplastic elastomer that molds to the perilaryngeal anatomy on insertion — creating a seal without the need to inflate a cuff balloon. This eliminates one step and one potential failure mode compared to inflatable-cuff SGAs. The gel-like material gently conforms to individual patient anatomy, reducing pressure-related mucosal trauma.

The i-gel Resus Pack is the EMS/emergency version of the i-gel, distinguished by its protective cradle packaging (which maintains the device's anatomical shape in transit and serves as a lubrication base), included lubricant sachet, airway support strap, and 12Fr suction tube. The 12Fr suction tube passes through the i-gel's integrated gastric channel, allowing gastric decompression without removing the airway device — important for reducing aspiration risk during prolonged resuscitation.

Size selection is by patient weight: Size 3 for 30–60 kg (small female, smaller adults), Size 4 for 50–90 kg (average adult), Size 5 for 90+ kg (large adults). Each size is color-coded for rapid identification. The airway support strap secures the device in position during resuscitation and transport, preventing displacement during CPR compressions and patient movement.

CoTCCC Context: The 2024 CoTCCC airway update evaluated EGA (supraglottic airway) versus BVM ventilation in combat trauma. Evidence found that SGAs were not superior to and may be inferior to BVM for non-cardiac-arrest combat trauma patients. Without RSI (rapid sequence intubation), EGA placement requires a deeply comatose patient. CoTCCC continues to support BVM as the primary ventilation strategy, with EGA as an option in specific circumstances authorized by the medical director.
