What is the King LTS-D Supraglottic Airway? The King LTS-D™ (by Ambu, formerly King Systems) is a disposable laryngeal tube supraglottic airway for positive-pressure ventilation and spontaneously breathing patients, with a unique dual-lumen design providing a separate drain tube for gastric access. Available in Size 3 (Kit, KLTSD423, patient height <61 in.), Size 4 (Kit, KLTSD424, patient height 61–71 in.), and Size 5 (Kit, KLTSD425, patient height >71 in.). Kit includes the LTS-D device, gastric diverter, 60cc syringe, 18F gastric tube, and sterile lubricant. Advanced provider only.

Product Overview
The King LTS-D is a widely used supraglottic airway in EMS, critical care transport, and emergency medicine. The "LTS-D" designates the Laryngeal Tube Suction–Disposable variant, which adds a separate drain tube lumen through the device — allowing gastric decompression by suction catheter without removing the airway from the patient. This differentiates it from the basic King LT (which lacks the gastric channel) and makes it the preferred variant for prolonged airway management where aspiration risk is a concern.
Insertion technique is simple and fast: the King LTS-D is inserted blindly into the hypopharynx/esophagus. With a single inflation port inflating both the oropharyngeal and distal esophageal cuffs simultaneously, ventilation is achieved through the ports between the two cuffs — directly over the laryngeal inlet. Positive pressure ventilation against cuffs inflated above 30 cm H₂O is supported, making it suitable for moderate-pressure ventilation scenarios.
Size selection is based on patient height rather than weight (unlike the i-gel), which can be measured or estimated. Size 4 is appropriate for the majority of average adult Americans (height 61–71 inches). The included gastric tube and diverter component allow gastric access for decompression or medication administration without airway removal. The color-coded sizes and single-inflation design support rapid deployment in emergency conditions.
